Residency is defined as the physical residence at which the student actually lives, keeps their personal possessions, sleep, and intends to remain. Students are assigned to the campus in the attendance zone they live and can only have one residence.

At the time of registration, parents must bring proof of residency and ownership showing the parents/guardians name and street address. No copies or notary services are available.

2018-2019 forms are now available.

The form below is used if the parent does not have a lease or mortgage statement in their name and resides with someone in the Friendswood ISD attendance zone.  The following documents are required along with the completed form:
xxxx• Parent’s Photo I.D.
xxxx• Student’s Birth Cert. or other acceptable proof of identity
xxxx• Resident’s Photo I.D.
xxxx• Current Utility Bill with service address listed (gas, water or electric –  disconnect notices or phone bills will not be accepted)
xxxx• Current Lease or Mortgage Statement (deed or GCAD statement may be substituted)
Only one form per student

Grandparent After School Care Affidavit

The Grandparent Care form may be used only if the Parent lives outside the FISD attendance zone and the Grandparent live within the FISD attendance zone. The following are required along with the completed form:
xxxx• Parent’s Photo I.D.
xxxx• Parent’s Current Utility Bill with service address listed (gas, water or electric – disconnect notices or phone bills will not be accepted)
xxxx• Student’s Birth Cert. or other acceptable proof of identity
xxxx• Grandparent’s Photo I.D.
xxxx• Grandparent’s Current Utility Bill with service address listed (gas, water or electric – disconnect notices will not be accepted)
xxxx• Grandparent’s Current Lease or Mortgage Statement (deed or GCAD statement may be substituted)
xxxx• The campus may request a copy of the Parent’s lease or mortgage statement to verify they reside outside attendance zone.

Affidavit of Minor’s Bona Fide Residence

Custodial Responsibility application is used if the parent is signing over their educational rights to another person. (ex. family, friend, neighbor)
*This affidavit does not take away from the custodian rights in any way, but only grants educational rights to the custodian and gives the campus permission to discuss the student. The following are required with the completed form:

xxxx• Student’s birth certificate or other acceptable proof of identity
xxxx• Resident’s Photo I.D.
xxxx• Current Utility Bill with service address listed  (gas, water or electric –  disconnect notices or phone bills will not be accepted)

The Board delegates to the Superintendent the authority to accept or reject any transfer requests, provided that such action is without regard to race, religion, color, sex, disability, or national origin
